Raynor Dental’s Patient Care Loyalty Program is not an insurance plan, but a payment arrangement provided by Raynor Dental PLLC for services rendered. Memberships for the program are provided exclusively to uninsured patients of our practice. Member patients are electing to prepay annually for preventive care instead of paying at the time of each service. Payment for any non-included preventive or restorative treatments is due at the time of service. Membership dues cannot be paid with CareCredit and must be current in order to receive services. Member patient discounts may not be combined with any other offers or discounts. It is the member patient’s responsibility to schedule and keep their appointments. No refunds will be provided for dues paid under any circumstances, including failure to schedule and maintain appointments. A member patient’s agreement is with the legal entity of Raynor Dental PLLC. Annual dues and fees for dental services may change at any time.
